You should consult a licensed physician, psychiatrist, psychologist, or other qualified medical or mental health professional for the diagnosis and treatment of any and all suspected health issues, mental health concerns, or emotional conditions that you may have before undergoing any hypnotherapy session, coaching, or related services. Any recommendations you follow regarding your health, emotional wellbeing, stress management, or lifestyle are entirely your responsibility.
In agreeing to work with Harmony Hypnotherapy, I hereby affirm that I am at least 18 years of age (or the legal age of majority in my jurisdiction), of sound mind, and legally capable of entering into this agreement on my own behalf. As such, I accept all risk of physical, emotional, or psychological injury to myself that might result from such participation and hereby release Harmony Hypnotherapy, its officers, agents, employees, and associates — including Sarah Mitchell, Certified Clinical Hypnotherapist — from any liability to me, my personal representatives, estate, heirs, next of kin, and assigns for any and all claims and causes of action for loss of or damage to my property and for any and all illness, injury, emotional distress, or psychological harm to myself including death, that may result from or occur during my participation in hypnotherapy sessions, group programs, coaching calls, online content, or any related services, whether caused by negligence of Harmony Hypnotherapy, its officers, agents, employees, and associates, or otherwise.
My participation in hypnotherapy and related services is strictly voluntary. No one is forcing me to participate, and I elect to participate in spite of the risks. I further agree to indemnify and hold harmless Harmony Hypnotherapy, its officers, agents, employees, and associates from liability for the injury or death of any person(s) and damage to property that may result from my acts or omissions while participating in the described and agreed-upon hypnotherapy session(s), group program(s), or coaching engagement(s). This includes causes which are known or unknown, specifically mentioned or implied, or not mentioned nor implied, which might exist or be claimed to exist at or prior to the date of this document.
The undersigned further specifically waives any claims or right to assert that any cause of action or claim or demand has been, through oversight or error, intentionally or unintentionally omitted from this release.
The undersigned also understands that Harmony Hypnotherapy, its officers, agents, employees, and associates — including Sarah Mitchell — provide hypnotherapy, coaching, and educational services, but are not licensed as medical doctors, psychiatrists, psychologists, licensed professional counselors, licensed marriage and family therapists, or licensed clinical social workers. Also, when the terms "hypnotherapist," "clinical hypnotherapist," "coach," or similar titles are used, they do not refer to state-licensed psychological, psychiatric, medical, or clinical professionals. Hypnotherapy services provided by Harmony Hypnotherapy are not intended to replace medical treatment, psychiatric care, psychotherapy, or any other form of licensed mental health treatment.
I certify that I am fully capable of participating in hypnotherapy and related services, and I have informed Harmony Hypnotherapy in writing (via intake forms, application forms, or direct communication) of any medical, psychological, or psychiatric conditions, medications, disabilities, or treatment history I may have that could reasonably affect my participation. I understand that I am responsible for disclosing:
Any current or past mental health diagnoses, including but not limited to depression, anxiety disorders, post-traumatic stress disorder, dissociative disorders, psychotic disorders, or personality disorders
Any history of suicidal thoughts, self-harm, or suicide attempts
Any psychiatric hospitalizations within the past 12 months
Any medications I am currently taking, particularly those affecting mood, cognition, or consciousness
Any history of seizures, epilepsy, or other neurological conditions
Any pregnancy or serious medical conditions
I understand that withholding this information could affect my safety and the effectiveness of the work, and that Harmony Hypnotherapy reserves the right to decline to work with any individual whose circumstances fall outside the scope of hypnotherapy practice.
I further understand and acknowledge:
Hypnotherapy is not a substitute for medical or psychiatric care. If I am currently under the care of a licensed medical or mental health professional, I will continue that care and will not discontinue any prescribed treatment or medication without consulting the professional who prescribed it.
Results are not guaranteed. Hypnotherapy outcomes vary from person to person, and Harmony Hypnotherapy makes no promises, representations, or guarantees of any specific result, outcome, or benefit.
In case of emergency, I will contact emergency services. If I experience suicidal thoughts, severe psychological distress, or a medical emergency at any time during or after working with Harmony Hypnotherapy, I will immediately contact emergency services (911 in the U.S.), the 988 Suicide and Crisis Lifeline, or go to the nearest emergency room. I understand that Harmony Hypnotherapy is not a crisis service and cannot provide emergency mental health intervention.
Session recordings and confidentiality. I understand that what I share during sessions is treated confidentially, but that Harmony Hypnotherapy may be legally obligated to report disclosures involving imminent harm to self or others, abuse of minors or vulnerable adults, or as otherwise required by law.
Therefore, I assume full responsibility for myself, for bodily injury, emotional distress, psychological harm, death, loss of personal property, and expenses (including all medical and mental health expenses) thereof a result of those inherent risks and/or my participation in hypnotherapy, coaching, group programs, or any related services provided by Harmony Hypnotherapy.
I have read, understood, and accepted the terms and conditions stated herein and acknowledge that this agreement shall be effective and binding upon me, my heirs, assigns, personal representatives, estate, and all members of my family.
Payment: All sessions, programs, and services must be paid for in full at the time of booking unless a payment plan has been specifically agreed upon in writing with Harmony Hypnotherapy.
Cancellation: Clients must provide at least 24 hours' notice to cancel or reschedule any scheduled session. Cancellations made with less than 24 hours' notice may be charged the full session fee at the discretion of Harmony Hypnotherapy.
No-Show Policy: Failure to attend a scheduled session without prior notice will result in the full session fee being charged.
Refunds: Due to the personal and time-intensive nature of this work, all payments for individual sessions, group programs, and packages are non-refundable once services have begun. Refund requests made before services begin will be considered on a case-by-case basis.
Program Commitments: Clients enrolling in multi-session packages or group programs commit to the full program duration. Discontinuing the program early does not entitle the client to a refund for unused sessions.
All content provided by Harmony Hypnotherapy — including but not limited to session materials, audio recordings, worksheets, videos, written materials, program content, and the "10-Minute Calm Your Mind Audio" resource — is the intellectual property of Harmony Hypnotherapy and Sarah Mitchell.
Clients are granted a personal, non-transferable, non-exclusive license to use these materials for their own personal development. Clients may not reproduce, distribute, share, sell, or publicly post any content provided by Harmony Hypnotherapy without express written permission.
By using the Harmony Hypnotherapy website, you agree to use the site for lawful purposes only. You may not use this site in any way that causes, or may cause, damage to the site or impairment of the availability or accessibility of the site.

Harmony Hypnotherapy reserves the right to modify these Terms & Conditions at any time. Changes will be posted on this page and are effective immediately upon posting. Your continued use of the website or services after changes are posted constitutes your acceptance of the revised Terms.
